About the role

Our dynamic Porirua Community Mental Health Team, part of the Mental Health, Addictions & Intellectual Disability Service, are seeking a resourceful, and enthusiastic NZ Registered Health Professional – Registered Nurse, Social Worker, Occupational Therapist.

If you are passionate about participating in people’s mental health and addiction recovery journeys, not afraid of hard work, well organised and keen to be part of a very dedicated group of professionals we have a vacancy for a motivated and enthusiastic registered mental health professional to join our integrated multi-disciplinary team.

You will be working with an experienced team who strive to provide high quality care to consumer’s that may require access to mental health and addiction services. Duties

  • Provide comprehensive assessments and plan support collaboratively with the client/patient, their family/whānau, clinical provider and Non-Government Organisation services.
  • Assist the client/patient and his/her family/whānau to achieve optimal health, well-being and safety through provision of appropriate assessment, education and support plan. 
  • Participate in, and contribute to, professional development activities in appropriate discipline.
  • Role model and provide best practice and cultural considerations in the multidisciplinary team process. 
  • Work closely with community agencies to achieve the outcomes agreed by the multidisciplinary team process.
  • Coordination and facilitation between the DHBs and multiple agencies.
  • Reports operationally to the Team Leader and is professionally accountable to the Professional Leader for the applicable discipline

Skills & experience

  • Current Professional registration and practising certificate - occupational therapist, social worker, or registered nurse.
  • Excellent clinical skills
  • Legally eligible to work in NZ
  • Full Drivers Licence
  • Experience in working from a strengths perspective
  • Proven experience in working with people with Mental and Addiction health issues
  • Good communication skills
  • Good clinical documentation skills.
